22% kidney func. Diabetic,need antidepressant

Question
I have been a diabetic for many years and for the past 12, have been insulin dependant.  I had a CVA in 11/08 in the cerebellum.

I am recovering, I am told better than many. My kidneys are now, however at around 22% (would have been nice if my doc had told me that they were problematic) instead, focus was on my liver and yet it is doing fine despite Hep C.  When I had my, actually two strokes on both sides of my cerebellum, a special "kidney harming dye was used.  This worsened my already poorly functioning kidneys. Due to the procrastination and misdiagnosis of my stroke symptoms in the critical early warning time, the angiogram doctor at another hospital had no choice but to use the dye.  Thankfully, what was found was not as terrible as my MRA had looked.  Yet, I had 2 strokes and am recovering.  I had been depressed for some time before this due to unfortunate happenings in my life.  My general practitioner, who never mentioned my kidney condition, had me on 75mg Wellbutrin 2 times daily.  This went on for well over one year.  It made me feel much better.  After all of this, my wife asked a prudent pharmacist to check all meds that they had put me on after the stroke, she emphasized to research Wellbutrin.  He responded that it is very contraindicated for kidney failure.  My general doctor switched me to Lexapro. It does not help me like the Wellbutrin did.  I'd like to know if a smaller dose of this will be o.k. I am hovering near dialysis, I know and this med will help me to feel better.

Answer
Hi Rich,

Thanks for your question on "Allexperts".

I am sorry to hear that you have been through a lot.

I agree with your pharmacist. Bupropion (Wellbutrin) is indeed contraindicated in severe kidney disease. It is NOT okay to take even smaller doses of this drug. Please ask your primary care physician to refer you to a psychiatrist.

Sincerely,

Dr. Shah
