Question
Hi Dr. Shah.
I am greatful for your answer last time about the time frame of the kidney biopsy. I am attaching a needle biopsy report of the kidney of my brother. According to the report, his diagnoses are glomerular obsolescence, tubular atrophy, and vascular changes accelerated on benign hypertrophy. According to renal palen II took on August 9, 2009, his serum creatinine, BUN, phosphorus, potassium, and bicarbonate level are 5.8, 61, 6.3, 6.3, and 12.9 respectively. His latest creatinine level is 4.9 and potassium is within normal limits.
Can you kindly guide me now should we go for dialysis now, whether the function of the kidney will be normal after a hemodialysis with this condition of kidney. Will his kidney function be normal after a dialysis or does he need frequent dialysis. In the report, they have mentioned that IF study was not done, does this give specific details regarding the function or percentage of damage of the kidney.
Every one at home are tensed sir. Please advice me.
Regards
Ashok BK
Answer
Hi Ashok,
Thanks for your question on "Allexperts".
I am afraid I don't remember the context of your question. Please understand that in addition to evaluating patients, I also answer questions on this website. It is not possible to remember details of each and every patient.
A light microscopy kidney biopsy report without immunofluorescence (IF)offers limited scope for interpretation. From the numbers you have provided and the LM report, there appear to be no salvageable elements in the kidney.
Please understand that dialysis is not going to cure your brother of the condition that caused kidney damage in the first place. It is a supportive mechanism for the body to survive in the absence of kidney function. If he has CKD, he will need dialysis three times a week in the foreseeable future.
Please discuss these details with your treating Nephrologist.
Sincerely,
Dr. Shah
